Check In
Your forms will be collected at our reception desk. We will make a photocopy of your insurance card and driver’s license. The front office coordinator will alert the clinical staff that you have arrived and are waiting to be seen. If you are unable to download the required forms prior to your appointment please plan to come in 30 minutes earlier than your scheduled time to complete all forms.
For your first visit you should plan to be here from 1½ to 2½ hours. Your eyes may need to be dilated at this visit. If you are uncomfortable driving with your eyes dilated, you may want to bring a driver. You are always welcome to bring a family member or friend to your appointment and are encouraged to do so.
During your visit an ophthalmic technician will review your medical history and medications with you, perform a vision screening and examine your eyes. You will then meet Dr. Price, and she will briefly examine your eyes to determine what, if any, special testing will be needed. Upon completion of these tests, she will review the results with you and will perform a more comprehensive eye examination. After the examination, she will explain her findings to you and will formulate a plan of treatment including any needed follow up. She welcomes your questions and encourages you to be proactive in your medical care.
